The Chosen One Global Evangelistic Ministry (TCOGEM) and its related organizations, such as Lazarus Muhammad Foundation (collectively, “the Ministry”), is committed to the highest standards of legal, moral, and ethical conduct. As part of fulfilling this commitment, TCOGEM requires all employees to report concerns regarding activity that may be illegal, unethical, or violate the TCOGEM Christian Code of Conduct, the Employee Handbook, or any of our other policies. We will use “Misconduct” below in relation to those concerns.
Additionally, TCOGEM encourages anyone to report Misconduct relating to TCOGEM or any of its employees, directors, officers, volunteers, or contractors. Our goal is to ensure that the Ministry avoids even the appearance of impropriety, and we appreciate your help.
This form is for official use only and is to be used to facilitate the reporting of violations in accordance with TCOGEM policy. The policy applies to activities of the Ministry, or any person associated with or doing business with the Ministry, and provides for the reporting of misconduct violations while ensuring confidentiality and protection against retaliation.
Do not use this form to report an immediate threat to life or property. If you require emergency assistance and it is safe to do so, please contact the appropriate local authorities.
If you are unable to submit this form electronically, you may download this form and submit via email at compliance@tcoglobal.org or mail to Compliance, Legal Department. The more information you can provide, the better our investigation will be. Please report whatever information you have at this time and follow up if you later receive additional information. We will consider it alongside other information collected.
You may provide information anonymously, but if you do so, please be aware that if we require further information to assess your complaint, we will not be able to contact you, and therefore, may not be able to act on your report.
If possible, please include information that answers the following questions. Wherever “Misconduct” is used below, we will treat this as a “suspected Misconduct.” The goal is to understand your concern clearly. An appropriate review of the reported circumstances will take place to determine actual wrongdoing.
What exactly did the person, company, or organization do that you are concerned about?
Where did the Misconduct take place?
When did the Misconduct happen?
Who committed the Misconduct (name of person, company, or organization), who may have been a victim, and who may be a witness?
Why did the person, company, or organization commit the Misconduct?
Please also include information about any other potential tips, documents, or evidence that may be helpful.
If the information we receive is not detailed enough for us to investigate, or if we cannot assess the merit of the information, we may postpone taking action until additional information is reported.
Upon receipt of your information, TCOGEM may take any one of the following actions:
Open an investigation; or,
Refer the matter to TCOGEM Human Resources for further review; or,
Refer the information to the specific TCOGEM ministry vice president which the report references.
If you have provided your identity, unless you are contacted directly by one of our investigators or other staff, there will be no communication from our office to you except for correspondence that may advise you that your matter has been referred to TCOGEM management.
The information you provide will be processed according to our internal policies and guidelines, and appropriate action will be taken pursuant to those policies and guidelines. The decision to take action relating to a report through an investigation or management referral is made in consultation with TCOGEM senior leadership.
